Client Support Specialist– Care Campus

The Client Support Specialist– Care Campus position at St. Joseph Center in Los Angeles, us is a full-time role. ABOUT THE ROLE The Client Support Specialist (CSS) is a highly visible, participant-facing member of the Care Campus team who proactively engages participants, identifies needs, connects people to services, and supports continuity between participants, Case Management, Safety, and other programs. WHAT YOU’LL DO Maintain an active presence in participant-facing areas and build respectful, trusting relationships using trauma-informed and person-centered approaches. Help participants understand available services and provide warm handoffs to case management, healthcare, behavioral health, hygiene, benefits, housing, and other resources. Support outreach, appointment reminders, document collection, follow-up, service navigation, and off-site accompaniment when appropriate.
